To enroll in WebView, you must be an established patient, provide us with your email address, and pay the subscription fee. Enrollment in WebView is for one year. Your cost will be $75 for yourself or $125 for yourself and your family. If you leave the care of Kachina Family Practice due to transfer or dismissal, a portion of your payment will be refunded on a pro rata basis.

Enrollment Online:

Kachina Family Practice has a secure online credit card payment system. You can use our website to enroll yourself or your entire family online. You can also re-enroll in WebView using our website.

First time enrollment
If you wish to enroll in Web View, the link below will take you to our secure payment site. After your payment has been verified, you will be able to print a receipt of your payment. After we have verified your enrollment information, you will receive an email containing your user name and password, and user instructions that will allow you to begin using Web View.

Re-Enrollment
If you wish to re-enroll in Web View, the link below will take you to our secure payment site. After your payment has been verified, you will be able to print a receipt of your payment. Your user name and password will remain the same.

Enrollment by fax, mail or in person:
